We would like to invite you to complete our annual Patient Experience Survey.
Your feedback helps us understand what is working well and where we can improve, so that we can continue to provide the best possible care and support when you need it. The survey is being run as part of a wider programme across West-Hill Health PCN to help practices understand patient experiences and identify opportunities to improve access to services.
The survey is:
- Anonymous
- Takes no more than 5 minutes to complete
- Open to patients aged 14 years and over
- Used to help improve services both within your GP practice and across West and North London
